Why this inspection matters#
Trench cave-ins kill workers fast — OSHA notes that a single cubic yard of soil can weigh as much as a car, nearly 3,000 pounds, and a wall can fail in under a second, leaving no time to move. That is why 29 CFR 1926.651(k)(1) requires a competent person to inspect excavations, adjacent areas, and protective systems daily and before the start of work, as needed through the shift, and after every rainstorm or other hazard-increasing occurrence — and only a competent person, who by definition can identify the hazards and has the authority to pull workers out on the spot (1926.650(b)).
The protective-system rules are just as firm. Under 1926.652(a)(1), any trench 5 feet or deeper needs a protective system — sloping, benching, shoring, or shielding — matched to the soil type, unless it is cut entirely in stable rock; at 20 feet or deeper, the system must be designed by a registered professional engineer (1926.652(b)(1)(iv)). Spoil piles, equipment, and material stay at least 2 feet from the edge (1926.651(j)(2)); a safe means of egress is within 25 feet of any worker in a trench 4 feet or deeper (1926.651(c)(2)); and where hazardous gases could accumulate, the atmosphere is tested before entry (1926.651(g)(1)).
Any FAIL stops entry. A trench is not entered until the competent person has cleared it.
How to use this checklist#
Fill in the date, inspector, location, and equipment ID. The competent person completes this before work begins each day, and again after rain, vibration, or any change in conditions. Mark each item Pass, Fail, or N/A, with specifics in the notes column. Any failed item keeps workers out of the excavation until it is corrected. The daily record is signed by the competent person before entry.

Corrective actions taken#
Describe any issues found and the steps taken to correct them. Where the competent person finds evidence of a possible cave-in, protective-system failure, or a hazardous atmosphere, exposed workers are removed from the excavation until the hazard is eliminated.
